Plain tee shirt with optional name heat seal. This is an example of using a variant to add a fee; the dropdown is a Shopify variant, and a Product Customizer small text field is used.

If you would like your name heat sealed, please select "Yes" for "Add Name?" (additional $4.99).

If you select "Yes", enter the name to print in the text field. If you select "No", any text in the text field will be ignored.
